Utility
Customer Service has made several forms available online for customer
use. Most of these can be printed out and mailed back to us. Customer
service surveys can be completed online.

Automatic
Payment Plan: For customers who want to pay their bills
by bank draft. |

Customer
Service Surveys: Customers who have received service
over the phone or in the field can respond online to let us know
how well we served them. |

Deposit
Waiver: You may request a deposit waiver through only one
of the following two ways:
- (Reference): Use this form if you are requesting waiver based on a prior
pay history with another water utility.
- (Credit
check): Use this form to authorize the City of Houston to
check your credit to see if you qualify for a deposit waiver.

Leak
Adjustment: Customers who have suffered and repaired an
excusable defect in their plumbing may apply for an adjustment of up
to three consecutive bills. Available in both English
and Español. |

Exemption Application Form: Customers who are sixty (60) years of age or older, low-income disabled or a veteran with a service related disability may be entitled to a waiver of any penalties for late payment and the fee for a re-read of the meter. |

Sewage Collection Application: Fill out this document if you are a Lake Houston customer who uses a sewage holding tank. |

Tap Inspection Contractor Application: Fill out this document if you have been approved by the City of Houston to set water tap. |

Unusually Large Bill Application: City of Houston ordinance allows residential customers to receive a partial reduction of a bill for unusually high consumption for which there is no known explanation. Only one bill in a 12-month period may be adjusted. Available in both English and Español. |
